A coroner stated that a motor driver died on a flooded road, with drains blocked and a temporary flood warning was a sign of five years.
James Scott, 25, Kings Worthi, Hampshire K, was declared dead in a two-vehicle collision scene at A33 near Winchester in July 2024.
In a report sent to the Hampshire County Council and the National Highway, Koronor Rosmond Rhodes-Camp said more regular maintenance could have cleaned water.
Both officials said they were considering their reactions.
Mr. Scott was driving south on BST around 21:45 on July 5, when he hit the car and the water standing near the horses pub.
His Vaxhall Corsa crossed the central reservation and collided with an upcoming Ford Ranger truck, said Koroner.
One in Future death prevention reportHe said: “It was observed in the examination that two drains near the south -east lane were narrowed with vegetation, resulting in very little or no surface drainage.
“This is happening on 19 May 2023 and 15 November 2023 despite the annual prescribed maintenance for the year 2023/2024.
“The forensic examiner concluded that the permanent water contributed to the collision.”
Ms. Rhodes-Camp questioned why there was only a temporary signal for so long.
He said: “While the exact cause of the flood is unknown, a more regular maintenance schedule may stop the manufacture of vegetation within the gullas.”
The Hampshire County Council and the National Highway have an 18 September to respond to the coroner.
In a statement, the council said: “We are very sad with this sad incident and our views remain with the loved ones of Mr. Scott in this very difficult time.
“We are considering our response to the coroner’s report and are unable to make any further comment while continuing this process.”
National Highways said: “Our views are with James Scott’s family and friends after this sad incident.
“We will review the coroner’s report and respond within the time limit.”
