A teenager, who was left alone in a mental health hospital, should have been under constant supervision when she died and later died, an inquiry heard.
On 14 February 2022, Ruth Szimkivicz was being taken care of by a member of the staff in his first innings at Hunterkomb Hospital near Medenhead.
Assistant Coronor Ian Wade KC said that he left work without entrusting a colleague to his responsibilities, said Ruth, Solisbury, to visit the hospital alone and go to his room, the assistant coroner Ian Wade KC said.
The man had false papers and flew back to Ghana, where he was believed to be from, and Ruth was later found unconscious. Two days later, he died at John Radcliffe Hospital in Oxford.
Ruth was looked after by a psychiatric intensive care unit (PICU) Temes ward in the hospital since October 2021. He was found out of a dining disorder.

The jury heard that she had climbed Kilimanjaro at the age of 11 years. She lived in Tanzania for a time with her mother and father, a GP and surgeon and her two sisters.
