A teenage girl who died after escaping from a police vehicle and was being able to remove her handcuffs on another car on M5.
17 -year -old Tamzin Hall died As he was being taken into a custody suite In November 2024, an Avon and Summerset Police car by two officers in Bridgewater.
A report from the Independent Office for Police Conduct (IOPC) has been published earlier, expanding how he removed his handcuffs and climbed the passenger seat in front of the police car from the back passenger side seat, which was parked on the side of the motorway.
Then he left the car, the accident climbed on the barrier and was hit by a car on the opposite carriageway.
Ms. Hall was arrested on suspicion of attack and criminal damage and was being transported from Toonon to Bridgwatter to a custody suite through M5.
The police car was on the car north side of the motorway, just before 23:00 GMT on 11 November.
The IOPC report, which does not name the Ms. Hall, says: “A female child was arrested for attack and criminal damage.
“He was placed in the back passenger side of a marked police car and handcuffed with hands posted in front of him. An officer was seated next to him during transport.
“On the way to detention, the child managed to remove his handcuffs, climb the front passenger seat, reach the front passenger door and get out.”
‘Facked with deadly injuries’
The report said that after crossing Carizway on the south side, he was declared dead on the spot after a hit.
The hearing of an inquiry at Wales Town Hall later that month was told that Ms. Hall, who was a student, was a constant of malignant head, neck and chest injuries.
IOPC is investigating his death, in which both officials, including contact, had done before dying with MS Hall.
The investigating officers are investigating “assessment of tasks, decision -making and risk of situation and whether it follows relevant training and policies”.
